Chondromas are benign tumors of mature hyaline cartilage. They are common in the long bones of the body but rarely reported in head and neck in the literature. Based on their location, chondromas can be classified as: enchondromas, periosteal chondromas and extra-skeletal or soft tissue chondromas. In this paper, we present an extremely rare case of recurrent periosteal chondroma occurring in the left premolar region of mandible of a 38-year-old woman. The diagnostic work up has to consider biopsy and CT-scan as well as clinical and radiological features. Chondrosarcoma has to be seriously discussed as a differential diagnosis. Long term follow-up of the lesion is mandatory because of recurrence high risk with controversial possibility of malignant transformation.
